Job Summary
Seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a rehabilitation and long-term care setting. The ideal candidate will thrive in a fast-paced environment, deliver patient-centered care, and collaborate effectively with interdisciplinary teams.
Key ResponsibilitiesProvide direct patient care under the supervision of an RN or provider
Administer medications and treatments accurately and safely
Monitor vital signs, patient conditions, and report any changes promptly
Assist with wound care, mobility support, and ADLs
Document nursing care in accordance with facility policies and regulatory standards
Support multidisciplinary care plans and communicate effectively with staff and families
Ensure a safe, clean, and therapeutic patient environment
Active and unencumbered LPN license (state-specific)
Minimum 6–12 months of recent LPN experience preferred
Strong clinical assessment and documentation skills
Ability to work 12-hour night shifts consistently
Reliability and ability to work scheduled days (Sun–Tue)
Compliance with facility onboarding and health requirements
