Nurse Practitioner (NP) or Physician Assistant (PA) – Part-Time Role | Champaign, IL

We’re expanding our high-quality clinical team and seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our Physical Medicine and Rehabilitation (PM&R) group—also known as Physiatry—serving patients in Champaign, Illinois (61821) .

This part-time position (12–24 hours per week) offers $70–$80+ per hour , with malpractice insurance covered and a collaborative physician provided for support. This opportunity is ideal for a motivated clinician who values flexibility, independence, and patient-centered care.

Your Responsibilities

• Perform comprehensive assessments for patients in skilled nursing and post-acute care settings
• Collaborate with physical and occupational therapists to design and implement individualized rehabilitation plans
• Evaluate patient progress, identify barriers to recovery, and adjust care as needed
• Communicate with nursing staff, therapy teams, and facility administrators to coordinate care
• Document all encounters accurately using Doctor Now, integrated with PointClickCare
• Support patient success and prevent rehospitalizations through proactive management

Qualifications

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Illinois
• Strong assessment, diagnostic, and communication skills
• Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred
• Independent, organized, and committed to delivering patient-centered care
• Professional and collaborative approach to team-based medicine
