Location - Flagstaff, AZ
Position: 1 Full-Time
Schedule: Flexible; options include 7:00 AM – 3:00 PM, 8:00 AM – 4:00 PM, or 8:00 AM – 5:00 PM, Monday – Friday. Some weekends may be required.
Overview:
We are seeking a Licensed RN or LPN to serve as a Quality Assurance Nurse , overseeing staff education and infection prevention. This role ensures compliance with regulations, evaluates care quality, and provides education to maintain high standards of resident care.
Implement and monitor policies and procedures for quality assurance using regulations, audits, and evidence-based practices.
Analyze data from audits, interviews, and reports to ensure compliance across all disciplines and departments.
Review monthly CASPER reports and develop QAPI-based Performance Improvement Plans (PIPs) as needed.
Prepare and interpret reports, statistical analyses, and data summaries for staff.
Assist managers in corrective action plans and monitor deficiency corrections.
Ensure compliance with state, federal, and VA regulations .
Investigate incidents of non-compliance, abuse, neglect, or mistreatment of residents.
Coordinate facility surveys and internal committee meetings, including Quality Assurance and Infection Control Committees .
Identify staff training needs and implement educational programs for quality and compliance.
Conduct infection control surveillance and track infections using McGeer and non-McGeer criteria .
Stay current with CMS, CDC, and other regulatory guidelines.

Nursing home regulations (OBRA and other applicable standards)

Active RN or LPN license in Arizona
DPS Fingerprint Clearance Card
Infection Preventionist Certification
