Stonehenge of American Fork is seeking an experienced MDS Coordinator to join our team. The MDS Coordinator is responsible for Case Management, MDS 3.0 Assessments, Care Area Assessments, Care Planning, Insurance updates, Medicare Part A Certifications, and Quality Reporting in an efficient and professional manner. Come help make Stonehenge of American Fork a “place of healing.”
