Wound Care Nurse (RN or LPN)

Location: Flagstaff, Arizona
Schedule: Monday–Friday, 8-hour shifts (No Weekends)

Shift Options: 7am–3pm, 8am–4pm, or 8am–5pm

Job Summary

The facility is hiring one Wound Care Nurse , either an RN or LPN, to oversee wound prevention, wound management, ostomy care, and related clinical responsibilities for residents in a long-term care/skilled nursing setting. This role serves as a clinical expert and educator for nursing staff and collaborates closely with physicians and interdisciplinary teams.

Key Duties
  • Perform wound assessments, reassessments, and ongoing wound care per policy and best practices.

  • Develop and implement comprehensive skin care programs to reduce skin breakdown.

  • Assist in developing and updating policies for wound care, wound prevention, ostomy care, tube/fistula management, and incontinence.

  • Update and revise care plans based on resident response and overall wound healing progress.

  • Assess and reassess resident pain; apply appropriate pain management strategies and educate residents/families.

  • Provide in-services and training to nursing staff on wound prevention, wound management, ostomy care, and incontinence care.

  • Communicate clearly and professionally with nursing staff and physicians.

  • Collaborate with interdisciplinary teams to address resident needs holistically.

  • Ensure all documentation meets current regulatory and practice standards.

  • Attend training and continuing education to stay current on wound care best practices, advances, and new treatment modalities.

Requirements
  • Active Arizona Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.

  • CWOCN (Wound, Ostomy, and Continence Nurse) or CWCN (Certified Wound Care Nurse) certification required.

  • Ability to obtain and maintain an Arizona DPS Fingerprint Clearance Card .

  • Current CPR card obtained through a physical, in-person assessment.

Selective Preferences
  • Experience working in a skilled nursing facility or long-term care environment.
