Job Description

**Responsibilities** The **Clinical Pharmacist** provides comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. The **Clinical Pharmacist** is responsible for medication order review, pharmacokinetic monitoring, drug information provision, patient education, and participation in clinical initiatives. + Review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies. + Monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity. + Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals. + Educate patients on their medications, including proper administration, potential side effects, and drug interactions. + Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases. + Participate in antimicrobial stewardship programs to promote the appropriate use of antibiotics and prevent antimicrobial resistance. + Monitor for and report adverse drug reactions. **Qualifications** + Completion of a 4-year course at an accredited School of Pharmacy with a BS or Pharm. D. degree + At least two years hospital pharmacy experience preferred or has completed an ASHP accredited residency. + Microsoft Office, Common Drug Information Software. + Current CA Pharmacist Licensure required. + Fire Safety card within 90 days of hire. **Overview** Founded in 1955, Dignity Health - Northridge Hospital Medical Center is a 394-bed, acute care, nonprofit community hospital located in Northridge, California.
